Child Health & Disability Prevention

What We Offer

The Child Health and Disability Prevention (CHDP) Program provides complete health assessments for the early detection and prevention of disease and disability in children and youth, offered at no cost to families that meet eligibility criteria. Health assessment services include:

  • Vision test
  • Hearing test
  • Dental assessment
  • Immunizations (shots)
  • Nutritional assessment
  • Developmental assessment
  • Lab tests for anemia, blood lead poisoning and tuberculosis
  • Health education

Additionally, your local CHDP office assists families with making medical appointments, arranging transportation resources, and referrals to other resources. The frequency of the exam schedules are determined by the state CHDP program as recommended by the American Academy of Pediatrics. Your local Child Health & Disability Prevention (CHDP) program does not provide direct services. 

The program does:

  • Encourage families with low and moderate incomes to seek preventive health care for their children
  • Help children get well-child exams, including school health exams and sports physicals
  • Assist families with children who need healthcare for problems identified during the CHDP exam
  • Recruit new providers and conduct site visits to assure that the providers give quality well-child exams
  • Look for trends and work with the community to address specific health risks

Your local CHDP office certifies doctors’ offices and clinics as CHDP Providers. Learn how to become CHDP Provider. CHDP provides free well-child exams for low-income children and youth in California.

Program Eligibility

  • Children with full scope Medi-Cal, without a share of cost, from birth through 20 years
  • Low- to moderate-income children from birth through 18 years
  • Children in Foster Care
  • Children in Head Start & State Preschool programs

For Parents

Preventive health exams can help children stay healthy, find health problems early, and answer many health questions. Through the CHDP program, eligible children and youth receive periodic preventive health exams. Children with suspected problems are then referred for diagnosis and treatment. Many health problems can be prevented or corrected, or the severity reduced, by early detection and prompt diagnosis and treatment.

How to Become a CHDP Provider

To apply to participate as a health assessment provider in the CHDP program, qualified individuals and entities must apply to the local CHDP program in each local jurisdiction in which they wish to render services. Educational Brochures

The CHDP program has numerous health education materials available to Lake County CHDP providers. Topics include Nutrition, Lead Poisoning, Safety, Immunizations, Dental Health, Tobacco, and Teen Health. The health materials are free of charge and can be ordered by using our Health Education Form available by calling the CHDP program.

Overview Training

CHDP conducts Overview Training to help CHDP providers and their staff successfully implement the CHDP program in their office. All staff involved with CHDP clients are welcome to attend. The training provides an overview of the CHDP Program, incorporating the patient assessment, documentation, tobacco assessment/counseling, dental classifications, and billing (PM 160) aspects of the program.

Injury Prevention

Unintentional childhood injuries are the leading cause of death and disability for young people. Falls and motor vehicle collisions (including bicycle and pedestrian) are the leading cause of hospitalized and fatal injuries. The Childhood Injury Prevention Program (CIPP) conducts countywide projects aimed at reducing unintentional injuries among children.
